The appointment form
The only form on this site is the appointment request form, found on the Contact page and at the bottom of the homepage. It collects three things: your name, your phone number and the reason for your visit, and nothing else. This information is used for a single purpose: responding to your appointment request. It is not used for marketing and is not shared with anyone beyond what is described here. Please do not write medical details into the form; your condition is discussed at your appointment.
Legal basis: your explicit consent, given with the checkbox above the submit button. You may withdraw it at any time by contacting us.
Where your data goes
Form submissions are handled by the site’s hosting provider, Netlify, which processes and stores them on servers located abroad. Sending the form therefore involves a transfer of your personal data outside Türkiye, which is why the form asks for your explicit consent. Each submission is emailed directly to the clinic’s inbox (cenkteker@gmail.com); the only other copy is held in the site’s hosting dashboard, where it can be deleted, and periodically is.
Like every web host, Netlify’s servers also record technical access data for each page request (your IP address, date and time, the page requested, browser type) in server logs; these are used only to run and secure the site and are kept by Netlify for a limited time.
Cookies and analytics (Google Analytics)
This site uses a single measurement tool, Google Analytics, and only with your explicit consent. Until you choose “Accept” in the cookie banner shown on your first visit, no cookie is set and no request is sent to Google; if you choose “Decline”, the site keeps working entirely without cookies and nothing about it is restricted.
If you accept, Google Analytics sets two cookies: _ga and _ga_LYM6M57R6V (both kept for 2 years). They are used to measure visits in aggregate (pages viewed, device and browser type, approximate location); they are not used to identify you. The measurement data is processed on Google’s servers abroad; accepting also covers this transfer of your data outside Türkiye (KVKK Art. 9).
You can change your choice at any time: the “Cookie settings” link at the bottom of every page reopens the banner, and choosing “Decline” deletes the analytics cookies and stops measurement. Your choice itself is remembered in your browser’s local storage (not a cookie), so the banner does not return on every page.
The map and WhatsApp
The maps on the Contact page and the homepage load from Google Maps together with the page; as with any content loaded from Google, this tells Google your IP address, and Google may use its own cookies inside the embedded map. The WhatsApp links open WhatsApp, whose own privacy terms then apply.
Your rights
Under Article 11 of the Turkish Personal Data Protection Law (KVKK, Law 6698), you may ask us: whether your data is processed; for a copy of it; for correction, deletion or destruction; to object to processing; and to be compensated for damage caused by unlawful processing. Requests can be sent to cenkteker@gmail.com and are answered within the legal time limit. You may also complain to the Turkish Data Protection Authority (KVKK).
