Noses differ between peoples as much as faces do. A Middle Eastern nose often has a high bridge, a prominent hump and a tip that droops when smiling. An African or East Asian nose may have a lower bridge, a wider base and softer cartilage under thicker skin. Mediterranean and Balkan noses have their own patterns. None of these are problems to be corrected; they are the starting point, and ethnic rhinoplasty is the name for surgery that refines a nose within its own character rather than replacing it with a standard shape.
Istanbul sits between Europe, the Middle East and Central Asia, and Dr. Teker has operated on noses from all of them for more than twenty years. His patients come from Türkiye and abroad, and he consults in Turkish, English, Arabic and French. That range of faces is the best training a rhinoplasty surgeon can have.
Refine, do not erase
The most common regret after rhinoplasty is a nose that no longer matches the face around it: too small, too narrow, too upturned for the cheekbones and lips it sits between. Dr. Teker plans every ethnic rhinoplasty against the proportions of your own face. A hump can be softened without flattening the profile; a tip can be lifted without becoming pointed; nostrils can be narrowed without looking pinched.
Skin and cartilage decide the technique
Two things make ethnic rhinoplasty technically different. Thick skin does not show the fine shape of the cartilage beneath it, so definition has to come from building a stronger framework rather than from removing tissue. Soft cartilage cannot hold a new shape on its own, so it is supported with grafts of your own cartilage from the septum, the ear or, for a low bridge, a small piece of rib. Dr. Teker does not use silicone implants; your own tissue is safer over a lifetime.
The Middle Eastern nose
A frequent request is the combination of a large hump, a drooping tip and a nose that lengthens when smiling. Lowering the hump alone leaves the tip hanging, so the tip is lifted and supported at the same time, and the muscle that pulls it down on smiling is released. The result keeps the strength of the profile while removing the heaviness.
Patience with swelling
Thick skin swells for longer. The bridge looks final within months, but the tip continues to refine for twelve to eighteen months as the skin shrinks around its new support. Taping after surgery and, occasionally, small injections help this along. Dr. Teker explains this timeline at the consultation, so that the slower course of the tip is expected rather than a worry.

How the operation is done
How the operation is done
- 01
Heritage-aware analysis
Dr. Teker measures your nose against the proportions of your own face, not against a European template. The plan keeps what is characteristic and refines what is not in balance.
- 02
Skin and cartilage assessment
Thick skin and soft cartilage change the technique. The plan focuses on building strong support so that definition shows through the skin.
- 03
Structural approach
The open technique is usually used. Cartilage from the septum, the ear or the rib is placed to raise a low bridge or to support and define the tip.
- 04
Reshaping
A hump is lowered, the bones are narrowed if needed, the tip is lifted and refined, and the nostril base is reduced when it is wide.
- 05
Skin management
Where the skin is thick, the soft tissue under it is thinned carefully and, after surgery, taping and occasionally small injections help the swelling settle.
- 06
Closing and splint
Fine stitches close the incisions and a splint protects the nose for a week.
Recovery, step by step
Recovery, step by step
- Day 1
Splint, congestion and mild discomfort as after any rhinoplasty.
- Week 1
Splint and stitches removed on day 7. Taping of the tip may continue for a few weeks to help thick skin settle.
- Weeks 2–4
Bruising gone, back to work. The tip is still noticeably swollen with thicker skin.
- Month 3
The bridge looks final. Tip definition is emerging but continues to improve.
- Months 12–18
Thick skin takes longer to shrink around the new framework. The final definition appears in the second year.
A note on risks
The general risks of rhinoplasty apply: bleeding, infection, anaesthetic reactions, numbness, minor irregularities and the possibility of a touch-up. Thick skin brings a longer period of swelling and a small risk of scar tissue forming in the tip, which is managed with taping and injections. Grafts used to raise the bridge can very rarely shift or become visible through thin skin. Dr. Teker explains what definition is realistic for your skin before surgery, so that expectations and anatomy match.
FAQ
Questions patients ask
Will I still look like myself?
That is the aim. Ethnic rhinoplasty refines the nose within the proportions of your own face and keeps the features that make it yours. It does not aim to give everyone the same nose.
Why does thick skin matter so much?
Thick skin hides the shape of the cartilage beneath it and swells for longer. To get definition, the framework under the skin has to be built stronger, which is why grafts are used more often.
Where does the cartilage come from for a low bridge?
From your own body, usually the septum or ear and, for a significant augmentation, a small segment of rib. Dr. Teker prefers your own tissue to implants for long-term safety.
Why does the final result take longer?
Because thicker skin shrinks slowly around the new shape. Most of the change is visible within a few months, but the last definition of the tip appears over 12 to 18 months.
